Work/Life Benefits

Vacation: 15 days first full calendar year, 20 days second year; accrued by the end of the year.

Personal Days: 3 days per full calendar year.

Holidays: 10 for 2008.

Family and Medical Leave: Up to 12 weeks job-protected leave for certain family and medical reasons.

Financial

Savings Plan 401(k): voluntary salary deferrals up to $15,500 pre-tax limit/year plus 25% firm match on 15% of deferrals. Also, firm contributes from 3% to 5% of gross annual wages regardless of voluntary deferrals.

Flexible Spending Accounts: Medical expenses not covered by the Medical Benefit Plan, such as eyeglasses, as well as the deductible and co-payment portions of the Medical Benefit Plan, are reimbursed up to $5,000 per year as pre-tax deductions. Health

Medical Benefit Plan: PPO plan through UnitedHealthcare. Coverage begins the first day of employment. In-network $20 co-payment. 100% in-patient hospital charges. Prescription drug program. Out of pocket maximum per calendar year for out-of-network expenses of $750 per individual or $2250 per family plus $400 deductible. Pre-tax monthly employee payroll contributions starting at $50/month based on annual salary and whether individual or family coverage. Medical benefits are extended to same sex domestic partners.

Dental: Aetna Freedom of Choice Plan offers DMO and PPO options. DMO – 100% coverage in-network for basic services and preventative services; 60% coverage for major restorations. No deductible. PPO – 80% coverage of preventative; 50% for basic and major services. $75 annual deductible for individual and $225 for family. PPO option allows in and out-of-network coverage not to exceed $1500 annual maximum after deductible for preventative, basic and major services. Plan contributions range from approximately $28 for individual coverage to $75 for family coverage. Coverage for orthodontia is provided under the DMO only, with benefits payable at 50%.

Annual flu shots on site at no cost to the employee.

Short-Term Disability Plan: full-pay based on length of service, from two weeks’ pay after three months to 26 weeks’ pay after five years of service.

Long-Term Disability Plan: 60% of salary at time of disability, paid after 26 weeks of short-term disability at no cost to the employee.

Long Term Care Insurance: Provides long term care (nursing home care, assisted living facility care or in home nursing care) to employees and eligible family members.

Voluntary Short-Term Disability and Critical Illness Insurance Plans.

MedjetAssist: Worldwide organization providing medical transportation, medical evacuation and medical consultation services if injured while traveling.
